How do I edit an already signed Medical Certificate of Death?

Question: I received a request to make changes to a Medical Certificate of Death (MCoD) I already signed and submitted using the digital workflow. Is it possible to make changes in Connect Care?

Context: A prescriber who completes a MCoD may be asked to make changes after the certificate has been signed and submitted. Requests may come from facility (hospital) wards, Health Information Management (HIM, which may be referred to as “registration” or “admitting”), funeral homes or Vital Statistics. If a MCoD is completed digitally in Connect Care, then any changes must be managed in Connect Care. 

Answer: If a need for minor corrections is identified soon after a MCoD has been completed and signed (i.e., within 2 days), it may be possible to simply “Modify” the existing form within the Deceased Navigator's "Cert of Death" section; this must be done by the same physician or nurse practitioner who created the original certificate. However, if major corrections or corrections to core demographic data (name, date of death, etc.) are needed, or if the need for change is identified more than 2 days after a MCoD was completed and signed, it is best to “Inactivate” the prior certificate and start a new one.
